The Chrysalis care home is located in North Surrey. A licensed residential care home for young, older adults with a mental illness. Chrysalis is supervised by the North Surrey Mental Team and all referrals are directed through the Centre.

The residents at The Chrysalis work towards independent living, therefore this home has a strong rehabilitation focus.

Who We Serve

The Chrysalis serves clients from North Surrey as well as other locations as supported by Fraser Health Authority. This type of care is for adults with complex mental illness who need further supports in order to optimize their independence and quality of life. Clients are stablized and require specialized rehabilitation services as they continue on their recovery journey.

A Place For Recovery

Research shows that specialized mental health services are best provided in smaller, more homelike facilities like Chrysalis. Clients have an individualized clients centered treatment plan. A specialized care team supports clients to gain the skills and abilities needed to transition back to their home communities with sufficient supports in place.


The focus of treatment is to help clients maintain stability, acquire daily living skills and achieve meaningful recovery. For example, clients receive the support necessary to participate in community programs and accomplish everyday activities provided by our experience staff daily. Chrysalis has highly qualified staff to meet the clients holistic need.

Objectives

To develop an individualized plan that is focused on the residents needs. In particular, resident's self-care, work and leisure skills. This plan will naturally also address the residents social and emotional needs.

To provide group programs and individual services for residents to work on their goals.

To enable residents to achieve their maximum level of independence.

The Program

Chrysalis is designed as a rehabilitation program for young and older adults with mental illness and other related problems. The resident's are between the ages of 19-70 years. This home is open to both male and female clients. The program offers a variety of activities including the following.

Activities are offered in home as well as outside the home in the community. Chrysalis maintains a close link with the local clubhouse, which the residents can easily access by bus. The clubhouse provides a variety of programs for the residents to develop their independent living skills as well as opportunities to explore leisure activities and learn new skills. Chrysalis provide the environment and experience necessary to allow each client to reach their maximum level of independence.

Areas in which residents may have experienced difficulties can be emotional, social, interpersonal and or vocational. This can result in poor stress management, interpersonal problems and at times difficulties managing their daily activities. At Chrysalis, clients are given the opportunity to work and gain back their lost skills so that they can live indepedently.

Staffing

Staffing

The Chrysalis has a Manager of Care (MOC) and experienced Nursing Staff who are Registered Psychiatric Nurses (RPN) / Registered Nurses (RN) with considerable hospital and community nursing experience. Chrysalis Nursing Staff are available from 0800 to 2000 during the day and evening. Health Care workers are available 24/7.

Direct services are provided on a twenty four hour basis 7 days a week. An activity worker organize activities, Monday to Friday with clients input.
